This book arose when the lead editor arrived to work in the Republic of Palau, an island nation situated in the tropical waters of the western Pacific Ocean, and realised there was no definitive modern text that covered the field of acute rheumatic fever and rheumatic heart disease. Determined to address this need, he procured a publisher and sought worldwide authorship. Most of the knowledge base pertaining to acute rheumatic fever and rheumatic heart disease originally came from the United States and United Kingdom in the 1930s-1950s.
