It has been over 7 years since the first edition of this book appeared. During that time it was well received all over the world and was translated into Portuguese and Chinese. I was humbled by the flood of appreciation notes from doctors, students, residents, and all those preparing for professional examinations. This has been a most gratifying feeling.
